Water Birth - Oh it's a baby!

I'm pleased to share some good news with you: Having your baby with fewer interventions is safer for you and baby in the overwhelmingly majority of births.  This is most likely to happen when you are cared for by a practitioner who believes in that philosophy whole heartedly and even more in a setting where that is the culture, like a birth center. In a new study of birth centers out today you can see some amazing statistics on how having your baby in one of the 79 midwife-led birth centers in one of 33 states.

One of the most remarkable findings in the Birth Center II Study was the cesarean rate was only 1 out 16 for participants who planned a birth center birth. This is compared to about 1 out of 4 low risk women in a hospital. (That rate of cesarean jumps to nearly 1 out of 3 for all women.)

So what does this mean?  The bottom line is that you can have a safe and healthy birth in a birth center if you're a low risk woman. In the end, not only will you personally experience these good outcomes, but as more women turn to safe care in birth center run by midwives, we will see cesarean rates fall and the costs of birth drop as well. (Just to name a few.)

P.S. Just because you go to a hospital that tacked on the words BIRTH CENTER to their wall, doesn't make it a birth center.  I urge you to read this definition of birth center. While the CDC says that only about 0.3% of US moms give birth in birth centers, I think that number is about to go up!

It might date me a bit to say that when I had my first baby, it was pretty much a Bruce Lansky baby name book or nothing. Lansky's tomes of names and meanings soothed my concerned pregnant soul that may baby would indeed one day have a name that: A) my husband and I could agree upon and B) would not send the baby to therapy.

I was tickled to hear that he had started a blog about baby names. While he's no long the only great name out there for baby names, his original thought process remains clear and crisp. 20 Week Ultrasound - 3D Hands

Second trimester ultrasounds, also known as mid-pregnancy or fetal anomaly screening, have been trying to be used to help detect Down Syndrome for years. There have been a variety of markers of Down Syndrome that can be identified via ultrasound, including:

  • dilated brain ventricles
  • absent or small nose bone
  • increased thickness of the back of the neck
  • an abnormal artery to the upper extremities
  • bright spots in the heart
  • 'bright' bowels
  • mild kidney swelling
  • shortening of an arm bone or thigh bone

Dr. Nicolaides and his team found the following findings did increase the risk of having diagnosis of Down Syndrome: "dilated brain ventricles, increased thickness of the back of the neck, and an abnormal artery to the upper extremities increase the risk by three- to four-fold, and an absent or small nose bone increases the risk by six- to seven-fold."

"The detection of any one of the findings during the scan should prompt the sonographer to look for all other markers or abnormalities," said Prof. Nicolaides. He added that the study also revealed that if a detailed second trimester ultrasound exam demonstrates the absence of all major markers, the risk of having a baby affected by Down's syndrome is reduced by more than seven-fold.

Knowing what weight each of the findings has can help parents decide if they intend to do more invasive testing, which can cause harm to the pregnancy or baby, like breaking the bag of water. Testing for Down Syndrome is not for everyone. Some people only want to know to act on if they should change providers or places of birth or have testing to alert them to other potential problems that may need immediate treatment, like heart conditions.

Someone on our Google+ page was asking if it's too early to register for baby stuff.  She's barely out of her first trimester. I briefly wondered if I was becoming one of those women who think it's too early for everything, but I did have a moment of "Why so early?"  Someone suggested to me that it was because moms are having not only more baby showers but having them earlier and earlier.

Your thoughts?  What's the ideal time for a baby shower?

One piece of advice that I've offered about what to do before you're ready for maternity clothes is to borrow button down shirts from dad. Many moms do this and love the flexibility is gives them as their clothing needs change during the day. (Read that as their belly seems to grow as the day wears on...)

So I went to a co-ed baby shower with a dear friend this weekend, she is also pregnant and was wearing her husband's button down shirt. The dad-to-be of this baby shower walked over and commented on the shirt saying that he knew that trick well. We started talking and he jokingly mentioned that sometimes, the shirt he wanted to wear that day was missing. He asked if I could tell all the pregnant women that I talked to, to please avoid taking the shirt that dad was planning to wear and particularly avoid his favorite shirt. This seemed like a reasonable request to me.

What about you? Are you a shirt stealer? Are you guilty of stealing the shirt of your man's back to cover your belly?

Michael Bublé and wife, Luisana Lopilato pregnancy, announced their pregnancy via a video message today. While the singer's son "Haven't Met You Yet" plays in the background you see their ultrasound and personal photos, announcing, "Mike, Lu...and mini Bublé!!!" The couple wed in 2011 and have been open about wanting a baby. From the looks of the ultrasound it's early days into Lu's pregnancy, but the heart rate looks strong.

The above photo is from the one and only nurse in that I've been involved with in my life. It was about a year ago at Target, after a mom had been asked to leave by a manager. It was a part of a larger group of nurse ins scheduled around the country. Even as I did it, I wondered, "Do nurse ins work?" Apparently, I'm not alone in my questioning, as you can tell by this article from Today's Moms.

Deena Blumenfeld, a Lamaze educator from Pennsylvania, offered this, "If somebody believes breast-feeding in public is inappropriate, you're not going to change their mind by breast-feeding in front of them," she said. "By putting it in someone's face, you're making it a confrontational issue and it shouldn't be, because it's normal."

While I hadn't thought of it that way, I can tell you that the group of moms that were with me, were all low key.  Most of us nursed at some point during the hour we sat, talked and shared stories of babies and breastfeeding. But I seriously doubt that unless you knew what we were doing specifically because you were there, you had no clue what was really up.  We looked more like a group of women, meeting at the local Starbucks within a Target store to chat.  To me, that's the best way to do it.

Shakira's baby boy is here!

It was announced on the singer's website in multiple languages: "We are happy to announce the birth of Milan Piqué Mebarak, son of Shakira Mebarak and Gerard Piqué, born January 22nd at 9:36pm, in Barcelona, Spain."

Earlier in the day a source had said that the baby was going to be born that day in a seeming induction of labor due to the "lateness of the pregnancy" and the "enormous" size of the baby. Milan, which means "dear, loving and gracious in Slavic; in Ancient Roman, eager and laborious; and in Sanskrit, unification," weighed in at 6 pounds 6 ounces. In my book, that's not a large baby by any estimation, in fact, it's smaller than the average size baby. Perhaps this was was sparked by the photo the couple released in the weeks before birth, with her gorgeous belly hanging out, which caused others to call her "huge."

So my take away points for you are to remember:

  1. It's nearly impossible to get an accurate read on how big the baby really is during pregnancy, even with ultrasound.
  2. Even if they think baby is large, it is not a reason to induce labor.

Have you ever been told you were having a big baby?  Was your baby as big as they thought? (Some babies are smaller, some are bigger according to these moms.)

It's a boy!

There is always a bit of discussion about who to share your pregnancy announcement with and when. I was about floored reading this story of one couple's experience of telling the wrong people about their pregnancy:

"We had waited almost until the end of the first trimester to tell people, but when we got together with my husband's friend and his wife, we were so happy that we couldn't help but tell them (plus I was showing quite a bit.). After they asked how far along I was, and learned that I was not quite 12 weeks, the wife said, 'God, there was a woman at my work who told everyone about her pregnancy in her first trimester and then ended up losing it. Wouldn't you feel like an idiot if that happened to you?' I was stunned speechless by her thoughtless insensitivity. By a cruel twist of fate, that very night when I went home, I started bleeding and the next morning an u/s confirmed the lack of a heartbeat. I learned some important lessons from this experience though: (1) always be gentle with others and resist giving opinions when people are going through challenges. (2) Choose carefully when you divulge sensitive personal info to people. Not everyone is worthy of your confidences."

<p><img class="alignnone" src="http://0.tqn.com/h/pregnancy/1/H/f/4/4/hospitaltourdoor1.jpg" alt="Hospital Tour Door #1" /></p> <p>The <a href="http://clk.about.com/?zi=1/1hc&zu=http://pregnancy.about.com/od/placeofbirth/a/hospitaltour.htm">hospital tour is something that some moms-to-be don't really give a second thought about</a>. You either do it and get wowed by the "show" or you skip it thinking it doesn't really matter anyway. Both attitudes are usually wrong.</p> <p>Let's address the show first. This is where they show a whole group of pregnant moms the biggest and best room. A room that usually has a fancy bed spread and is prepared just for the tour. This is the one bed spread the hospital owns. I can almost guarantee you that you won't see it when you show up to have your baby. Not all the rooms look like this one, they aren't as big and the furniture is not always as functional. (Think of that crazy wheeled grocery cart you always wind up with...) This room is a bit too perfect, and for good reason. It's not a typical room. Sure, the medical "stuff" is hidden and that's fine, it's the bling that gets me on these tours - the Laura Ashley bed spreads, the music, the birth ball. If you see it on your tour, ask what the chances are you'll see it when you have your baby. Remember, at the end of the day - the tour is a sales pitch and the nurse is a great realtor.</p> <p>Thinking of skipping the tour?  (Particularly after the above commentary?) Well, don't.  The tour is worth it. This is your chance to ask a lot of really good questions about how the hospital will function when you have your baby. The answers, and non-answers, can tell you a lot about the climate, particularly around topics that may be important to you, including baby care, pain relief options, water birth, rooming in, etc.</p> <p>May advice? To get the most out of the tour take people with you, even your doula if you have one. <a href="http://clk.about.com/?zi=1/1hc&zu=http://pregnancy.about.com/u/ua/placeofbirth/questions-to-ask-at-a-hospital-tour.htm">Prepare a list of questions to ask on the tour</a>, from the mundane (Which door should we come in at 3 a.m.?) to the specific (What's your policy for the number of people in the room at the time of birth?). Tour multiple hospitals, even if you are pretty sure you know where you will wind up. What chores do you hate while pregnant?

<p><img class="alignnone" src="http://0.tqn.com/h/pregnancy/1/H/9/n/3/iStock_000009073958XSmall.jpg" alt="Nesting before labor - pregnant woman" /></p> <p>That's the question I asked on Twitter and Facebook this week, mostly as a way to avoid my own housekeeping for a few minutes. The answers I got really surprised me:</p> <ul> <li>"...anything that means I have to get on the floor and get back up again (when heavily pregnant!)"</li> <li>"Cleaning out the fridge. *barf*"</li> <li>"Bathroom! The tub, the toilet, the floor! All of it. Yeah and bending and scrambling in tight places..."</li> </ul> <p>While there are a few examples of staying safe while cleaning in pregnancy, like avoiding certain chemicals, being in a well ventilated place, etc. Most of us are healthy enough to clean. Does Hospital Pre-resgitration Matter in Labor?

Many hospitals offer pre-registration for labor and birth. It usually involves a basic form that asks about your name, insurance info and a bit of information about your pregnancy like your due date, midwife or doctor's name. But many people diligently turn in these forms every year only to have experiences like Mig. Mig says,

"I pre-registered at the hospital, brought a copy of the pre-registration, my husband was standing there with our insurance cards and all that crap, and the admittance nurse STILL wouldn't admit us until I, personally, in the throes of labor, gave her my name, address, and insurance information about a million times while she typed away with two fingers on what a swear must have been one of the original IBM desktops. She kept pointing at some poor woman moaning behind a curtain, saying, 'She's been here 3 hours already. You have time to fill out this paperwork.' It was absolute justice when my water broke all over the reception furniture and her shoes.  My son was in my arms less than 2 hours later."
