Turkey Uses Law to Try to Curb Cesarean Rate

Layer of Muscle - Cesarean Section

A new law in Turkey is awaiting the signature of the president, but it's already passed parliament. The goal is help curb the country's cesarean rate which is over 65% in private and university hospitals. They have also tried reducing the payments made for cesarean births as a way to decrease the number of cesareans.

While it is still legal to do a cesarean birth, it is supposedly only to be done for the utmost of medical emergencies. This would hopefully reduce the rate of non-emergency cesareans, scheduled cesareans for non-medical reasons and the like. The first reaction tends to be shock that a government would do this, but when you step back and look at the truth of how we get people to change behavior, it's more often policy and pocket book than it is about education.

Are hospitals in your area trying to curb their cesarean rates?  (If you don't know what your hospital's cesarean rate is, you can find it here.)

About Pregnancy & Childbirth: 50 Shades of Baby

50 Shades of Baby

Every where I turn, people are talking about E.L. James' 50 Shades of Grey novel. Now, if you've been living in a remote location, with no access to the Internet, you may have missed this discussion. The story of a young woman who becomes involved with a filthy rich, slightly older, but handsome man, who just happens to have some strange proclivities involving physical relationships. (I'm trying to keep this blog post as "clean" and as safe for work as I can!)

Lots of books clubs are reading it. You see lots of people talking about it varying terms. Someone was talking about how they were predicting a 50 Shades Baby boom next year. I read a blog talking about how women were now interested in this genre because of these books. Are you going to to be considering baby names like Christian or Anastasia? Or are you skipping it in favor of some other summer reading or parenting books?

So, what's your prediction, a 50 Shades of Baby boom?

About Pregnancy & Childbirth: The World's Most Hilarious Birth Plan

Birth Plan

There is a hilarious link floating around to a birth plan entitled: Jamie and Jeff's Birth Plan. It's totally tongue in cheek, but absolutely hilarious at the same time. I'm sure that there are some labor and delivery nurses who have read things that probably sound very close to that one.

So what should your birth plan say?  Well, that depends on what you want, where you're planning to give birth and some other things. Though, I can tell you what a birth plan is not:

  • a contract
  • only for people who are planning a home birth
  • only for people who are planning a hospital birth
  • only for people who want an unmedicated birth
  • only for people planning for an epidural

The whole point of a birth plan is to open up communications with your practitioners and the people who are with you when you give birth. Sometimes circumstances dictate that writing your thoughts down make it easiest to communicate it to people with you and other times writing it down is purely for you as a way to solidify your thoughts.

Affordable Care Act and Maternity Care

Newborn hand

With the Supreme Court upholding the Affordable Care Act (ACA) yesterday, there are numerous provision that will alter the care for women in pregnancy and beyond.  Some of the big ones include that pre-exisiting conditions that are currently used against women, like a previous cesarean birth and being a victim of domestic violence, can't be used against them; extended coverage under medicaid for low and moderate income women to get basic maternity care; smoking cessation programs for pregnant women; expanded nurse midwifery payment; and others.  Here are some places to find out more about how ACA will change coverage in pregnancy:

It is also important to note that while some of these provisions have already taken effect, others will not go into effect until 2014.

About Pregnancy & Childbirth: Fear of Childbirth

Fear of Childbirth

Take Charge Routine

An interesting study out of Norway caught my eye proclaiming that when women feared childbirth, their labors were about 47 minutes longer than those who did not fear childbirth. My gut reaction was, "Of course it does!" The doula in me has seen this hundreds of times.

There were some interesting points in this study. The epidural rates in both groups were incredibly low by American standards, 26.8% in the no fear group and only 44.8% in the fear group. They also found that the fear group had a higher rate of use of forceps/vacuum extraction. They postulate that the fear increases catecholamines, stress hormones, and therefore prolongs labor. This is the basic premise that most people who talk about stress in labor point out, though some catecholamines in labor are expected.

Communication with the practitioner is something that they bring up saying, "...fear of childbirth may be associated with poor communication." Then they say that not being about to communicate with the careprovider may delay obstetric intervention and therefore delay labor. This may be more a product of the Norwegian system, because my take would be that the lack of communication may be part of the source of the fear to begin with, causing unwanted interventions.

I was pregnant at the same time that Olympic winning gymnast Mary Lou Retton had her first baby. I still recall watching her complete her routine three weeks after knee surgery, scoring a perfect 10, and thinking how horribly painful this must have been and how she described it as doing what she had to do to complete what she had trained for all those years. So when I listened to her talk about her fear of pain in labor, I was dumbfounded. How could she be afraid of labor after all of the aches and pains of being an Olympic gymnast? After all, her body was built for labor and it certainly was not built to withstand Olympic training. Listening to her made me realize that fear of labor wasn't the right way to look at it for me. So I came into my first birth with a healthy respect for labor, but not fear.

How do you manage fear or concerns about labor? That's where a good childbirth class, talking to your provider and a doula can all help. These all go to great lengths to address your personal fears about birth and parenting. Try not to feel overwhelmed. Finding the right support, which is sometimes professional, mental health support, can be very beneficial before labor.

Where do you fall in the healthy respect versus fear of labor scope?
