A study just published looks at the length of labor now, compared to 50 years ago. There were the obvious things that we hear about more often, that moms are older and heavier, but these researchers said that even looking at those factors, labor was still longer. Their study showed that modern obstetrics has a lot to do with why labor is longer - including the use of epidural anesthesia which adds about 40-90 minutes on the average labor. They even postulated that with the use of Pitocin as rampant as it is in modern obstetrics, an increase in 19% over the 1960s, labor might be even longer than the what the study found: a first time mom has a 2.6 hour longer labor than her 1960s counterpart and that a mom who has previously had a baby will have about two additional hours of labor in modern times.

One of the things that they hope will come from the study is that common labor practices will change. Perhaps the timing for the decision of a cesarean section (c-section) could be outdated, as many cesareans happen prior to 6 centimeters dilation. Women may simply need more time to labor. When asked how women should use this information, one of the study authors said that he thought well informed patients might want to seek out practitioners who understand the differences in labor patterns in modern labor. This again confirms that the choice of your doctor or midwife is crucial to how your birth is likely to be.

They also said that moms who want natural birth may be comfortable waiting longer to getting Pitocin and other labor interventions assuming everything else is going well in labor. Time alone means little when looking at old labor patterns, like the Friedman Curve used in many hospitals.

Chest Press for Pregnancy

One of the nice things about the chest press is that there are a variety of ways that you can do it. You can do it one at a time or alternating sides. What I like is using this as a great way to break up whatever I'm doing and make it exercise. This is great for when you're putting away the groceries - just insert a can of beans in place of the dumbbells and you're all set.

Having a worry about birth is fairly normal. Nick Cannontalked about his fear before the birth of his twin with wife, Mariah Carey.  Though it's not something that just celebrity dads suffer from - nearly everyone has some sort of anxiety about birth.  But this really got me thinking about birth and what we could do to help partners cope better in the delivery room.

I spend a lot of time talking to women about their fertility.  Many times women who want to talk about their fertility are coming to me after many months of trying to conceive.  They are concerned about their ability to get pregnant.  Thankfully the majority of these women simply need to make some minor modifications like better timing for sex, figuring out when they ovulate, etc.  Many women just need more time.

The lack of patience with our fertility is something that is frustrating for everyone.  I think it largely comes from being told all our lives that, when we didn't want to be pregnant, it would only take "once" to get pregnant.  While that is true, the proverbial once is the statistical anomaly. But it lulls us into a false sense of security about our fertility.  That "one time" fallacy is waiting for us when we're ready to conceive.  This leads us to panic when we've had sex way more than one time and we still aren't pregnant.

So what can you do to not take your fertility for granted?

All of this is designed to protect your fertility and to help you identify problems early on.  This is one of the best ways to not take your fertility for granted.

"What do you have at home?"  That's the how the ultrasound tech started the conversation when we went to a 20 week ultrasound with our second baby.  "A girl, " my husband responded.  She scanned quickly and announce, "Well, this is a boy!  You've now got the perfect family!"  That comment always struck me as an odd one.

Certainly many people would love to have "one of each," but I also know many families who are happy with all boys or all girls, or a mixture of each.  (The large family version of the "perfect family" is supposed to be an equal number of girls and boys.)  I just think it's different for every family and that there isn't one perfect.

Fifth Disease is something that is contagious and that can be serious for pregnant women.  Those at greatest risk include moms who have older children or who have jobs working with children, like day care workers, teachers, etc. The good news is that the vast majority of women may already be immune to Fifth Disease.

This week has been declared World Doula Week. The popularity of doulas has risen drastically in the years since I first heard the word.  I think that this is largely due to the influence of these birth assistants and their tireless efforts for moms and their families.

People choose doulas for all different reasons.  Sometimes it's about the benefits of having a doula, like a reduction in the cesarean rate or a shorter labor; sometimes it's because a practitioner or friend recommended a doula for a birth; or perhaps it's just what seemed like the right thing to do for you and your labor.

Tell us about your doula and what you love about your relationship.

Tori Spelling and Dean McDermott

Tori Spelling and husband, Dean McDermott welcomed a new baby girl, Hattie Margaret, on October 10, 2011.  Today she announced that baby number four is on the way. The couple also have children Liam and Stella.

I'm completely thrilled for her.  I love having a large family.  Tori will need to be careful to have great nutrition because of the stress on her body from having another baby so close to this one.  If she's still nursing Hattie, it is considered safe for her to nurse in pregnancy. Other than that, wow, having four is a delightful handful.  What advice would you give Tori?

Reese Witherspoon is expecting her third baby, according to US Weekly. She and husband Jim Toth got married last year in California. Reese is already mom to Ava and Deacon, with ex-husband Ryan Phillippe.

Researchers recently looked at the risks of losing a baby after amniocentesis and chorionic villus sampling (CVS) when a mother is obese.  They found that if a mother with a BMI of 30-40 had an amniocentesis or a CVS procedure, that there was no increase in the risks to the baby.  However, if a mother had a BMI of over 40.0, there were increased risks found with an amniocentesis, but not CVS.

While there is still a risk of losing a baby because of the invasive nature of both of these tests, the risk is very small and for most women, weight does not increase that risk. This may allow more mothers who would choose to have genetic testing done to do so without added worry of increased risk. And if you happen to want genetic testing and have a BMI over 40.0, perhaps a CVS test is better for you. This should be one of the many questions that you would ask before having genetic testing done.

What can you do about pregnancy cravings?

Pickles and Ice Cream

Pregnancy cravings are something that lots of women have while pregnant, and even a bit beyond. Certainly it is the stuff of many a cartoon and comic joke. But when you are the one who experiences the cravings, it's not so funny. But what's a mom-to-be to do about her cravings?

I know that I have certainly wanted to ask my husband to go get me something special from the store at an odd hour - but that's so cliche! I've tried ignoring the cravings.  I've tried to dissect them - why am I craving this?  Some of it helps, some of it doesn't.  I did find a few things that have worked for some of the cravings, and sometimes not so much.  What did you do?

Pregnant Woman Quadriceps Stretch

The quadriceps stretch can feel great at relieving tension in tight thighs in pregnancy. It's fairly simple to do and can really help loosen your muscles even before your exercise. One of the issues in pregnancy is the tendency for you to be too loose in the ligaments and tight in the muscles. Getting out and moving can help and stretching this large muscle group can help you warm up prior to other exercises like walking, running or swimming.

Baby Shower Diaper Game Label Each Diaper

This diaper game is one of the many versions out there. I used to avoid it like the plague when I was throwing a baby shower. But I braved it this weekend and was delighted with the results. We all had a blast. Sure, it's not right for all baby shower audiences, but a lively group or a coed baby shower - it's probably an appropriate idea, even if it is about a diaper filled with chocolate!
